 As the nurse educator, I would use this poster to educate the practice that this patient and patient like this above patient need to be educated on their diseases processes and the importance of remaining complaint. I would also create patients’ educational material that addresses the issues in this poster, for example, making pamphlets and hand-outs that would address the importance of patients making lifestyle changes and   maintaining those changes along with monitoring their diagnosis at home. These pamphlets and handouts would be patient friendly, easy to read and understandable (Agency for Healthcare Research and Quality, 2014). Communicating and collaborating with the unit leaders and nurses, placing emphasis on discharge education would be optimal so that patient teaching would be implemented and maintained. Since the nurses are the last persons to see the patient’s before they leave the hospital or the healthcare facility; I would educate and or remind the nurses that the patients should be given in-depth discharge teaching along with the pamphlets upon discharge. The pamphlets would also include additional websites where patients can access more educational materials.
